Eye Bag Removal In Singapore: What Non-Surgical Treatments Can And Cannot Do

Most eye bags are caused by fat behind the lower eyelid pushing forward as the tissue holding it back weakens with age. Non-surgical treatments such as tear trough filler, skin tightening and skin boosters can soften how noticeable a mild eye bag looks, but they cannot remove or reposition that fat. When the fat itself is the problem, lower eyelid surgery is the only way to remove it.

Eye bag removal in Singapore - what non-surgical treatments can and cannot do

What Are Eye Bags?

Eye bags are a bulge under the lower eyelid. They are easy to confuse with these three related concerns, and the difference changes the treatment.

ConcernWhat It IsTends To
Eye BagsFat behind the eyelid pushing forwardStay visible all day and gradually increase with age
PuffinessFluid collecting in the eyelid tissueWorsen in the morning, after salty food or allergies, and settle during the day
Tear Trough HollowingA groove between the eyelid and cheek from volume lossLook like a shadow or dark circle, sometimes making a mild bag look bigger
Dark CirclesPigment, visible vessels or shadowLook like a surface discoloration rather than a bulge

What Causes Eye Bags?

Ageing Of The Supporting Tissue

Behind the lower eyelid sits a layer of fat that cushions the eye. A thin membrane called the orbital septum holds it in place. Age-related degenerative changes in the orbital fat and septum commonly lead to sagging tissue, allowing the fat to bulge forward. These eye bags are frequently accompanied by tear trough deformity and, in some patients, flattening of the midface1. That is why eye bags and hollowing often appear together.

Genetics

Some people develop visible eye bags in their twenties because of the natural shape and fat distribution around their eyes. If your parents had prominent eye bags early, you may too.

Fluid And Lifestyle

Salt, alcohol, allergies, crying and sleeping flat can all make the eyelids puffier for a few hours. This is fluid rather than fat, so the puffiness comes and goes.

Medical Causes

Swelling around the eyes can occasionally signal a medical condition, such as thyroid eye disease, kidney problems or an allergy. If puffiness appeared suddenly, affects one eye only, or comes with redness, pain or changes in vision, it is advised to see a General Practitioner (GP) or go to the Accident and Emergency (A&E) before considering any cosmetic treatment.

How Do You Know If Your Eye Bags Are Fat or Fluid?

Comparison of under-eye bags caused by fat versus fluid

A few signs can sometimes suggest the underlying cause:

  • Puffiness from fluid is usually worse on waking and improves over the day. It changes with sleep, salt and allergies.
  • Fat-related bags stay much the same throughout the day and can gradually more noticeable over years.
  • Hollowing looks more like a shadow than a bulge and can change with lighting and head angle.

A doctor can diagnose this in person by examining the eyelid in different positions and lighting. This matters, because treating a fat bag as if it were fluid, or filling a groove beneath a large bag, can make things look worse.

What Non-Surgical Eye Bag Treatments Can And Cannot Do?

“Non-surgical eye bag removal” is a phrase many people search for. It is worth being clear about what it means in practice: non-surgical treatments reduce how noticeable eye bags look. They do not remove the fat.

TreatmentWhat It Can DoWhat It Can’t DoSuitable For
Tear Trough FillerFill the groove beneath a small bag so the transition from the undereye to the cheek is smoother and the bag looks less protrudedRemove or shrink the fat; it can make large bags look fuller if placed incorrectly or too much product usedMild bags with hollowing below
Energy-Based Skin TighteningImprove firmness of thin, lax under-eye skinReduce or remove fatEarly laxity and crepey skin
Skin BoostersImprove skin quality and thicknessChange the bulge itselfThin, crepey skin over a mild bag

Tear Trough Filler For Eye Bags

Filler does not treat the bag itself. It treats the hollow beneath it. In a patient with a small bag and a deep groove, softening the groove can make the bag far less obvious. In a patient with a large bag, filler can potentially add volume in the wrong place and can make the area look heavier.

Skin Tightening Around The Eyes

Energy-based treatments deliver controlled heat into the skin to encourage collagen production. Around the eyes, the aim is to achieve firmer, smoother skin. This can help early laxity, but it does not remove herniated fat.

Energy-based treatments options include Morpheus8 RF microneedling, Ultraformer MPT HIFU and Volnewmer radiofrequency. The most suitable treatment will be determined by the doctor based on your individual concerns and skin condition.

Improve Skin Quality For Undereyes

Skin boosters are designed to improve skin quality rather than remove eye bags. Depending on the formulation, they may help improve hydration, skin texture and the appearance of fine lines, making the under-eye area look smoother and more refreshed. They may be considered when the main concern is thin, dehydrated or crepey skin, rather than prominent under-eye bags.

When Is Surgery the Better Option?

If the bulge is caused by fat that has pushed forward, lower eyelid surgery (lower blepharoplasty) is the treatment that directly addresses it.

There are two main approaches:

  • Transconjunctival. The surgeon works through the inside of the lower eyelid. This approach avoids a visible external scar2.
  • Transcutaneous. An incision just below the lash line, used when excess skin also needs removing.

Traditional fat removal alone may worsen hollowing under the eye, which is why surgeons increasingly refine surgical methods such as repositioning the fat into undereye hollows rather than simply removing it1.

While Astria does not perform eyelid surgery. If your assessment suggests surgery would help more than non-surgical treatment, your doctor will explain this and can advise on your next steps.

What Are The Risks to Know About?

  • Filler under the eyes. Bruising and swelling are the most common effects. Less common risks include a bluish tinge from superficial placement (the Tyndall effect), lumps, prolonged swelling.
  • Energy-based treatments. Temporary redness and swelling that subsides within 1-2 days.
  • Skin boosters. Temporary swelling, redness, bruising and tenderness may occur at the injection sites. Depending on the product used, small bumps or unevenness may also occur temporarily, particularly in the delicate under-eye area.
  • Surgery. Swelling, bruising and, uncommonly, changes to the eyelid position, or hollowing if too much fat is removed.

Under-eye skin boosters$300 to $900 per session

What Happens At A Consultation At Astria?

A 3D facial analysis is performed before the consultation, with photographs taken to document your current skin condition and referenced during the doctor’s assessment.

The doctor examines your eyes in different positions and lighting, asks how the bags have changed over time and whether they vary through the day, and checks for anything that needs medical attention first. They will tell you whether your eye bags are mainly fat, fluid or hollowing, which options could realistically help, and whether non-surgical treatment is suitable or surgery would serve you better.

Frequently Asked Questions (FAQ)

No. Non-surgical treatments can make mild eye bags less noticeable, for example by filling the hollow beneath them or firming the skin, but they cannot remove the fat that causes true eye bags. Only surgery removes or repositions it.

It depends on the cause. For a small bag with a hollow groove beneath, tear trough filler often makes the biggest visible difference. For thin, lax skin, skin-tightening energy-based devices or skin boosters may help. A doctor’s assessment decides which is suitable.

Early eye bags are often genetic, due to the natural fat distribution around the eyes. Allergies, fluid retention and sleep can make them look worse, but they are not usually the root cause.

Eye creams may reduce temporary puffiness a little, but they cannot shift fat that has pushed forward. They are not a treatment for established eye bags.

Yes, if it is used in the wrong individual or if too much product is used. In patients with large eye bags, filler adds volume and can make the area look heavier. Experienced doctors will aim to use fillers to smoothen a hollow which will give a natural, refreshed look. That is why the cause needs assessing first.

The fat removed or repositioned does not usually return, so results are long-lasting. The skin and tissue around the eyes continue to age, however, so the area will still change over time.
